Application to Enrol
Applications for a place at Craigie Heights Primary School can be made at any time during the year. Enrolment of students is a two step process:
- Step 1 is to lodge an Application for Enrolment Form (Part A) with the school which can be downloaded here.
- Step 2. If your child is offered a place at the school, you will be required to complete and lodge an Enrolment Form (Part B) which can be obtained from the School Office.
As per the Department of Education’s Enrolment Policy, Part A must be accompanied by:
- your child’s birth certificate (telephone the Registry of Births, Deaths and Marriages on 1300 305 021 if you do not have one)
- your child’s Immunisation History Statement (for instructions on how to obtain this statement visit the Department of Health)
- proof of your current address:
- either a current lease agreement or a local government rates notice, and
- a recent electricity, water, telephone or bank statement (at least one)
- if applicable, copies of Family Court orders for confirmation of proof of name or custody arrangements.
- if your child was not born in Australia: evidence of date of entry into Australia, passport or travel documents, and current visa details (and previous visas if applicable). Temporary visa holders must also provide relevant enrolment permissions or visa documentation (e.g. Education International approval where required).
Submit your application for enrolment and all supporting documents to craigieheights.ps@education.wa.edu.au. Please do not hesitate to contact us on 6206 2200 if you have any questions.
